Engraved Flashing Tag

The size of the characters on the actual tag will change to fit the tag, becoming smaller as you use more lines and/or more characters per line. Lots of letters will make the print more difficult to read without getting very close to the tag, so keep that in mind as you fill in the form.
Letters will all be capitalized, and each line is centered on the tag. You do not have to use all of the lines, or all of the spaces in any line. The lines will not allow you to enter too many characters; they will stop at the listed number for that line.
Typical tags will have your pet's name on the first line, and your name/address/phone on the other lines, however you are free to use the lines for anything that will fit on them.
We are used to seeing intentional strange spelling, so please do not expect that we will change anything you type here just because it looks like it is misspelled! Check your information carefully before submitting.
